Eager to start your personal training regimen? No problem. A few simple steps and we'll be at your doorstep waiting to get you in shape!
1. Fill out the free Session form and return back here when you have completed it. You are entitled to a free fitness assessment and sample session which runs approximately 35-45 minutes. This is a great time to meet your trainer, see what he or she has to offer you, go over goals, answer any questions and check out their training style with a short sample session. This form gives us all the information we need to schedule you with a trainer that has availability for your schedule and that will best meet your individual specific needs. If you can't stand forms, give us a call at (845) 544-2472 and we get all the information we need from you over the phone.
2. Fill out the Personal Training Intake Form below. Prior to your first session with Jiva it is necessary that we gather information to better evaluate your condition. This information is completely confidential and will not be shared with any outside parties. To speed up your process time and allow more time for your first session we offer this form that may be filled out online so there is no need to interrupt your session with paperwork.
3. Your done! We'll be in touch soon via phone and/or email to discuss and confirm the days we were able to schedule your free session. We will also let you know the name of the trainer and their background at this time.
